I agree to receive life coaching and information for educational purposes from Heart Centered Living LLC provided by Sarah Dickey, who is a Heart-Centered Hypnotist, PPNE, yoga teacher, transpersonal life coach, and wellness consultant.
I fully understand, acknowledge, and accept that Heart Centered Living and Sarah Dickey DO NOT practice medicine or act as a licensed health care provider, doctor, psychologist, psychotherapist, licensed professional counselor or in any other health care or mental health care role, and I am not seeking medical diagnosis or treatment.
I understand that all services provided, and all information obtained during any consultation or coaching are purely educational and informational in nature and do not constitute medical care or mental health care.
I also understand that it is my responsibility to discuss any recommendations, advice, or information provided during consultations and coaching with my health care provider.
I have been advised to consult with my physician before I undertake any exercise program.
I, on behalf of myself and anyone claiming through me, release, discharge and hold harmless Sarah Dickey and Heart Centered Living LLC from any and all claims, causes of action or liability of any kind or nature and damages or loss which I may have arising out of or in connection with the services received.
I agree to indemnify and hold harmless and defend Heart Centered Living LLC and Sarah Dickey from any and all claims.
I assume sole responsibility for the decisions I make as a result of any consultation and coaching.
I have read and understand this Release and Waiver Agreement and agree to be bound by its terms. My agreement is entirely voluntary and my own informed choice.
